Querida María Ángeles,
Hoy, 9 de febrero de 2027, te escribo esta carta para contarte cómo
gestiono mi condición crónica, cómo interactúo con el sistema sanitario,
cómo participo en la investigación; para hablarte también del apoyo entre
pacientes, de los estándares e interoperabilidad y sobretodo de mi
registro vital de salud… Verás cómo las nuevas tecnologías ya existentes
en 2017 han acabado transformando los sistemas de salud, gracias al
cambio de mentalidad y roles de los actores principales (entre los que
incluyo al paciente). Tengo la esperanza de que esta carta nos haga ser
más rápidos en imaginar y diseñar un nuevo sistema de salud, y sobre
todo de aunar fuerzas para conseguirlo.
Tengo un registro de salud vital personal. Es un registro mío, no vinculado
a ninguna organización en particular, pero que puede interactuar con
todas en general. Es un registro que absorbe de forma automática todas
mis interacciones con el sistema sanitario público y con otros centros
privados, tanto de atención primaria, como de atención especializada.
Además, se cargan ahí todas las pruebas de laboratorio y de otro tipo,
independientemente de quien las realice. Esta información se carga de
forma inteligente (no solo en formato texto), y me permite ver, por
ejemplo, cual es la evolución de mi hemoglobina glicosilada a lo largo del
tiempo, relacionarla con otro tipo de variables y aprender de ello. Lo
tengo conectado con todo tipo de dispositivos: el de glucosa, por
supuesto, y otros que miden mi calidad de sueño, la actividad física y
todos aquellos parámetros que deseo conectar.
Tengo un medidor de glucosa continuo que envía información a la
plataforma cada cierto tiempo y cada vez que me inyecto insulina, el
propio bolígrafo/bomba envía también información sobre la hora y la
dosis inyectada. Si llevo más de dos días descontrolada, el sistema me
alerta y me sugiere tomar ciertas medidas de corrección. Como ves, este
registro de salud vital da sentido a todas esas APPs y dispositivos que
anteriormente estaban inconexos y potencia tremendamente su valor.
Cada vez que como, saco una foto del plato, indico su peso en gramos y
una APP calcula la cantidad y tipo de hidratos de carbono que estoy
consumiendo, además de sugerir la dosis de insulina que me debo
inyectar. Ya no hay sorpresas, como ocurría antes, que cada vez que comía
fuera de casa, era muy complicado calcular la cantidad de hidratos de
carbono que estaba consumiendo y la dosis de insulina que debía
inyectarme. Por supuesto, esta información también se carga en mi
registro de salud.
El registro de salud vital personal, ha sido posible gracias a la revolución
que se montó hace unos años. Cansados de que cada sistema sanitario o
empresa de seguros mantuviese su propio historial clínico, algunos
pacientes y agentes del ecosistema de salud en España, nos unimos para
generar una plataforma que pronto adquirió un carácter universal, segura
y en la que cada ciudadano pudo tener su propio registro integrado. Un
registro que nos aporta información de valor para nosotros, que nos
permite mantener o mejorar nuestro estado de salud y que nos motiva en
la adopción de hábitos de vida saludable. El pistoletazo de salida tuvo
lugar en la segunda jornada de la Asociación de Salud Digital celebrada en
Madrid el 9 de febrero de 2017, cuando una paciente que sonaba un tanto
extravagante y que decía venir de un Clúster de empoderamiento del
paciente en el Impact Hub de Madrid, leyó una carta desde el futuro a un
público altamente implicado en la transformación del sistema de salud.
Allá por el 2017 el sistema de salud público, los sistemas privados, las
empresas de seguros y algunos otros agentes, estaban empeñados en
construir y custodiar el mejor historial clínico electrónico para los
pacientes. Cada uno de ellos nos aseguraba que la información disponible
en el suyo era de vital importancia para nosotros, pero a menudo no la
entendíamos. Para convencernos de que su registro era el mejor,
permitían incluso introducir algunos datos provenientes de nuestras
interacciones con otros sistemas y empezaban a absorber datos de
algunos dispositivos y apps de salud; esas que empezábamos a utilizar con
mucha ilusión pero que la gran mayoría de nosotros dejábamos de lado,
aburridos, unos meses después de haberlas estrenado. Cada uno de esos
agentes pensaba que su historia clínica era la mejor, la que más
funcionalidad ofrecía, la que más seguridad proporcionaba y la que los
pacientes debíamos utilizar, ante todo. Eso sí, los datos ahí disponibles
eran tremendamente difíciles de extraer en formatos que no fueran papel
o PDF y la integración con otros sistemas, cuando era posible, era
realmente precaria y difícil.
La estandarización de datos y la interoperabilidad no cogió fuerza hasta
que empezamos a trabajar en el registro de salud vital universal. Hasta ese
momento a ninguna entidad le interesaba realmente que sus datos
pudiesen ser compartidos con otras instituciones. ¡Los datos son de los
pacientes! decían, pero la forma de registrarlos y extraerlos, no facilitaba
en absoluto la interoperabilidad que nosotros, los pacientes,
necesitábamos.
Ahora, al nacer, te crean tu registro de salud y se empiezan a volcar ahí
todos tus datos de salud. Se ha convertido en un registro universal, que
recoge información de millones de ciudadanos de todo el planeta, con lo
que hemos conseguido una masa crítica de datos tal que, gracias a la
inteligencia artificial, se ha convertido en una herramienta de inteligencia
colectiva. Algo realmente útil para la salud del ciudadano, para los
profesionales sanitarios, para la sostenibilidad del sistema de salud
público, así como para el mundo de la investigación.
Esta plataforma nos permite a los ciudadanos con enfermedades crónicas
y familias estar en contacto con otros de características similares a las
nuestras. El soporte entre pacientes en los diferentes estadios de la
enfermedad, es algo que los sistemas sanitarios finalmente han entendido
e incorporado como parte de muchos tratamientos.
Ya no tengo visitas programadas con el endocrino. Mis interacciones con
él, a menudo de carácter virtual, ocurren en momentos concretos, bajo
demanda, cuando surge una necesidad especial o cuando mis niveles de
glucosa andan alterados. Siempre utilizamos el mismo procedimiento: le
envío mis cuestiones previamente y en la reunión, vamos al grano. A veces
no es necesario ni la reunión, con los nuevos cursos de realidad virtual,
que me prescriben, he sido capaz de resolver muchas de mis dudas, de
recibir información sobre nuevos fármacos y dispositivos de nueva
generación.
A través de la plataforma podemos también ver un registro de todos los
ensayos clínicos que se están llevando a cabo a lo largo de todo el planeta.
La información, gracias a la realidad virtual, está presentada de forma que
la podamos entender. Y no solo eso, podemos también incidir sobre
cuáles son nuestras prioridades de investigación, aquello que nos aporta
verdadero valor. Podemos ceder nuestro registro histórico de salud y
participar en las investigaciones que deseamos. Hoy en día, la mayor parte
de los ensayos que se realizan, no se realizan con humanos, sino con
simuladores de fisiología humana. Las supercomputadoras y la posibilidad
de acceder de forma rápida a miles de registros de salud, entre otros, han
acelerado este proceso tremendamente y lo que antes se hacía en años
hoy se hace en pocos meses.
Hoy podemos decir, que tanto la medicina preventiva como la orientada al
diagnóstico y tratamiento, son realmente personalizadas. Nuestra
información genética y metabólica está ya cargada en nuestro registro de
salud. Parece mentira que hace tan solo diez años recibiésemos
tratamientos diseñados para la media (para todos, nos decían, aunque
realmente no estaban adaptados a casi nadie). Pero no solo eso,
últimamente se están incluyendo también características psicosociales
para diseñar nuestros tratamientos óptimos.
Pero lo que más me gusta de mi registro de salud vital, es mi “gemela
virtual” o “digital twin”. La primera vez que oí hablar de este concepto fue
allá por el 2015, y no era para humanos, sino para máquinas y grandes
infraestructuras. Se trataba de una tecnología utilizada para emparejar
objetos físicos a sus gemelos virtuales, permitiendo así hacer labores de
simulación, análisis y predicción de diferentes acontecimientos en los
gemelos virtuales. Unos años después se extrapoló ese concepto a un
gemelo digital personal. Así pues, mi gemela digital tiene intrínseca toda la
información volcada en la plataforma e interactúo a menudo con ella. Está
conectada con el resto de gemelos digitales, aprenden entre ellos y de
nosotros y esto nos permite a mis médicos y a mi tener acceso a mucha y
muy relevante información. Hacemos simulaciones de mi estado de salud
en el futuro e incluso comparo mi estado de salud y otra serie de métricas
con la de otras personas. Pero lo verdaderamente importante es que ella
me ayuda y me motiva a mantener un estilo de vida saludable. Al fin y al
cabo, ella es capaz de registrar y procesar datos que para mí es imposible
controlar y puedo centrar mis esfuerzos en vivir…
Seguro que a estas alturas estás pensando en la humanización, en que
todo este mundo digital, ha ido en contra de las relaciones humanas. Pero
no nos engañemos, tenemos ahora mucho más tiempo para las relaciones
humanas que hace diez años. Porque ¿era acaso humano, toda la
burocracia que teníamos que sufrir los pacientes? Las visitas programadas,
los desplazamientos, las esperas, pedir y recoger recetas… ¿era humano
que los profesionales sanitarios tuviesen que atender a más de 40
pacientes al día, tratando de mantener a flote un sistema de salud
diseñado para una realidad que no era la del momento?
Ahora, con el registro de salud vital, se han rediseñado y digitalizado
muchos procesos, se han eliminado muchas ineficiencias y duplicidades
intra y entre organizaciones sanitarias, el liderazgo compartido en salud es
una realidad y el sistema sanitario público y privado, vuelven a ser
sostenibles.
Por último, el nuevo sistema de salud del 2027 es mucho más que
reactivo, se trata de un sistema proactivo, enfocado en atender la salud de
todos los ciudadanos y la enfermedad de aquellos que la padecen. Por lo
tanto, abarca mucho más que la atención sanitaria y se extiende a nuevos
agentes, no estrictamente sanitarios.

Como veis, las cosas en el 2027 podrían ser muy diferentes. Pero no
olvidemos estas tres cosas:
 La tecnología disponible en el 2017, ya permite todo lo que
describo…
 Los principales retos son deconstruir mentalidades y ser capaces de
visualizar una realidad diferente sustentada en modelos diferentes…
 Nadie será capaz de liderar la transformación necesaria solo…
Tenemos la capacidad de convertir los actuales sistemas de salud en
unos mucho más inteligentes para todos.

Dear Maria Angeles,
Today, February 9, 2027, I am writing this letter to tell you how I manage
my chronic condition, how I interact with the health system, how I
participate in research; to let you know about the support among
patients, standards and interoperability and specially to introduce you to
my vital health record ... You will see how the new technologies already in
place in 2017 have ended up transforming health systems worldwide,
thanks to the mindset change and the transformation of the main actors’
roles (including the patient). I hope that this letter will make us faster in
imagining and designing a new health system, and especially in joining
forces to achieve it.
I have a personal vital health record. This record belongs to me; it is not
exclusively linked to any specific health organization, but it interacts with
all of them. It is a smart record that automatically absorbs all my
interactions with the public health system and other private centers;
primary as well as specialized care. In addition, all lab and other sort of
tests are loaded there, regardless of who performs them. This information
is loaded smartly (not only in text format), and allows me to analyze, for
example, what the evolution of my glycosylated hemoglobin is over time,
relate it to other indicators and learn a great deal from it. It is also
connected to all kinds of devices and wearables: those that measure the
glucose, of course, and others that measure, for example, the quality of
my sleep, my physical activity and all those parameters that I want to
connect.
My continuous glucose monitor sends information to the platform from
time to time and the pen/pump does it too every time I inject insulin.
Whenever my glucose is uncontrolled for more than two days, I receive an
alert and a suggestion to take certain corrective actions. As you see, this
vital health record gives sense to all those APPs and devices that were
previously unconnected and it remarkably potentiates their value.
Every time I eat, I take a picture of the plate with an APP; enter the weight
in grams, and considering the amount and sort of carbohydrates of the
meal, it suggests me the dose of insulin to inject/infuse. No more surprises
like before! Whenever I ate out, calculating the amount of carbohydrates I
was consuming and the amount of insulin I had to inject was hard and
often only a guess. Of course, all this information is also automatically
loaded into my vital health record.
The personal vital health record has been made possible thanks to the
mobilization organized a few years ago. Tired of the fact that each health
system and/or insurance company kept its own disconnected medical
record system, some patients and agents of the health ecosystem in Spain,
got together to design and build a platform that soon became universal,
safe and in which each citizen could have his own integrated record. A
record that provides valuable information to us, the citizens, which allows
us to maintain or improve our health condition and motivates us to adopt
healthy lifestyle choices. The kick-off took place at the second Digital
Health Association meeting, held in Madrid on February 9, 2017; when a
patient coming from a so-called Cluster for Patient Empowerment at the
Impact Hub Madrid and who sounded a bit odd, read a “letter from the
future” to an audience highly involved in the transformation of the health
system.
Back in 2017, public and private health systems, insurance companies and
some other organizations, were committed to building and guarding what
they called the best electronic medical records (EMR) for patients. Each of
them assured us that the information available in their EMR was of vital
importance for us, but we often did not understand the information in it.
Those EMRs even allowed us to introduce some data from our interactions
with other systems and started to absorb data from some health devices
and apps; those that we began to use with great enthusiasm but that the
great majority of us often put aside, bored, a few months after having
started with them. Each of these organizations thought that their EMR
was the best, offered the most functionality, more secure and the one
that citizens had to use primarily. Of course, the data available in those
EMRs was tremendously difficult to extract in a smart format (other than
paper or PDF), and the integration with other systems, when possible, was
precarious and difficult.
Data standardization and interoperability did not gain momentum until we
started working on the universal vital health record. Until then, no
organization was really interested in having its data shared with other
institutions. “Data belongs to the patient!” they said, but the way that
data was registered and extracted, did not provide at all the
interoperability that we, the patients, needed.
Now, every new-born baby gets his vital health record where health data
is registered from the beginning. The platform has now grown into a
universal platform that collects information from millions of citizens
around the world, which has achieved a critical mass of data such that
thanks to artificial intelligence, it is now a truly collective intelligence tool.
Something extremely useful for citizens, for health professionals, for the
sustainability of public health systems, as well as for the research world.
This platform allows chronic condition citizens and families to be in
contact with peers. Peer support in the different stages of life is
something that health systems have finally understood and incorporated
as part of many treatments.
I no longer have scheduled visits to the endocrinologist. My interactions
with him, often of a virtual nature, occur on demand when a special need
arises or when my glucose levels are uncontrolled. We always use the
same procedure: I send my questions in advance and we get down to the
point at the meeting. Sometimes the meeting doesn’t even take place,
with the virtual reality courses I get prescribed, I have been able to solve
many of my concerns and to receive information on new generation drugs
and devices.
The platform allows us to localize all the clinical trials that are taking place
all over the world. The information, thanks to virtual reality, is offered in a
way that we, citizens, can understand. And not only that, we can also
influence the research agenda based on our priorities and what brings us
true value. If desired, we can grant investigating institutions access to our
vital health record and participate in the research we want. Furthermore,
today, most of the trials are not performed with humans, but with
simulators of human physiology. Supercomputers and the capability to
rapidly access thousands of health records, among others, have
accelerated investigation processes tremendously and what took years in
the past, nowadays is done in a few months.
Today we can say that preventive medicine as well as diagnosis and
treatment, are truly personalized. Our genetic and metabolic information
is already loaded into our vital health record. It is difficult to believe that
only ten years ago, we received treatments designed for the average (for
everyone, they told us, although they were not really adapted to almost
anyone). But not only that, recently, psychosocial characteristics are also
being included to design our optimal treatments.
But what I like most about my vital health record is my "digital twin". First
time I heard about this concept was back in 2015, and it was not meant
for humans, but for machines and large infrastructures. It was a
technology utilized to match a physical object to its virtual twin, allowing
to perform simulations, analysis and predictions of all kinds at the virtual
twin environment. A few years later, this concept was extrapolated to a
personal digital twin. Thus, my digital twin has intrinsically all the
information loaded on the platform and I often interact with her. She is
connected to other digital twins, they learn from each other and from us,
and this allows my doctors and me to have access to plenty of interesting
and useful information. We can make simulations of my health in the
future and even compare my health status and other set of metrics with
others. But what's really important is that she assists and motivates me to
make healthy lifestyle choices. After all, she can record and process data
that is impossible for me to control simultaneously and I can focus my
efforts on living…
Surely, at this point you are thinking of humanization, and that this whole
digital world has gone against human relations. But let's be realistic, we
now have much more time for human relations than we did ten years ago.
Besides, was the bureaucracy that patients had to suffer those days
human? Programmed visits, travels to and from locations, waiting lists and
time, prescription requests and picking up procedures... was it human that
health professionals had to see more than 40 patients a day, trying to
sustain a health system designed for a reality that actually didn’t exist
anymore.
Now, thanks to the vital health record, many processes have been
redesigned and digitalized, many inefficiencies and duplications within
and between health organizations have been eliminated. Co-leadership in
health is now a reality, public health systems are again sustainable and
private ones are back on business.
Finally, the new health system in 2027, is not just reactive, it is a proactive
system, focused on addressing the health of all citizens and the disease of
those that suffer from it. Therefore, it covers much more than healthcare
alone; it extends its boundaries incorporating a complete set of new social
and industrial agents.

As you can see, things in 2027 could be very different compared to now.
But let us not forget these three points:
• The technology available in 2017, already allows everything described in
the letter...
• They main challenges are mindset change and being able to visualize a
different reality built on different models...
• No one will be able to lead the necessary transformation alone ...
We have the capacity to transform current health systems into much
smarter ones for everyone.

Carta de un paciente cronico desde el futuro / A cronic patient letter from the future

Carolina Rubio Miner leyó esta “carta desde el futuro” en la Jornada de la Asociación de Salud Digital el 9 de febrero de 2017.Gracias a todos los pacientes, profesionales y gestores sanitarios, profesionales de la industria y tecnólogos que me habéis inspirado… Querida María Ángeles, Hoy, 9 de febrero de 2027, te escribo esta carta para contarte cómo gestiono mi condición crónica, cómo interactúo con el sistema sanitario, cómo participo en la investigación; para hablarte también del apoyo entre pacientes, de los estándares e interoperabilidad y sobretodo de mi registro vital de salud… Verás cómo las nuevas tecnologías ya existentes en 2017 han acabado transformando los sistemas de salud, gracias al cambio de mentalidad y roles de los actores principales (entre los que incluyo al paciente). Tengo la esperanza de que esta carta nos haga ser más rápidos en imaginar y diseñar un nuevo sistema de salud, y sobre todo de aunar fuerzas para conseguirlo. Tengo un registro de salud vital personal. Es un registro mío, no vinculado a ninguna organización en particular, pero que puede interactuar con todas en general. Es un registro que absorbe de forma automática todas mis interacciones con el sistema sanitario público y con otros centros privados, tanto de atención primaria, como de atención especializada. Además, se cargan ahí todas las pruebas de laboratorio y de otro tipo, independientemente de quien las realice. Esta información se carga de forma inteligente (no solo en formato texto), y me permite ver, por ejemplo, cual es la evolución de mi hemoglobina glicosilada a lo largo del tiempo, relacionarla con otro tipo de variables y aprender de ello. Lo tengo conectado con todo tipo de dispositivos: el de glucosa, por supuesto, y otros que miden mi calidad de sueño, la actividad física y todos aquellos parámetros que deseo conectar. Tengo un medidor de glucosa continuo que envía información a la plataforma cada cierto tiempo y cada vez que me inyecto insulina, el propio bolígrafo/bomba envía también información sobre la hora y la dosis inyectada. Si llevo más de dos días descontrolada, el sistema me alerta y me sugiere tomar ciertas medidas de corrección. Como ves, este
  • 2. registro de salud vital da sentido a todas esas APPs y dispositivos que anteriormente estaban inconexos y potencia tremendamente su valor. Cada vez que como, saco una foto del plato, indico su peso en gramos y una APP calcula la cantidad y tipo de hidratos de carbono que estoy consumiendo, además de sugerir la dosis de insulina que me debo inyectar. Ya no hay sorpresas, como ocurría antes, que cada vez que comía fuera de casa, era muy complicado calcular la cantidad de hidratos de carbono que estaba consumiendo y la dosis de insulina que debía inyectarme. Por supuesto, esta información también se carga en mi registro de salud. El registro de salud vital personal, ha sido posible gracias a la revolución que se montó hace unos años. Cansados de que cada sistema sanitario o empresa de seguros mantuviese su propio historial clínico, algunos pacientes y agentes del ecosistema de salud en España, nos unimos para generar una plataforma que pronto adquirió un carácter universal, segura y en la que cada ciudadano pudo tener su propio registro integrado. Un registro que nos aporta información de valor para nosotros, que nos permite mantener o mejorar nuestro estado de salud y que nos motiva en la adopción de hábitos de vida saludable. El pistoletazo de salida tuvo lugar en la segunda jornada de la Asociación de Salud Digital celebrada en Madrid el 9 de febrero de 2017, cuando una paciente que sonaba un tanto extravagante y que decía venir de un Clúster de empoderamiento del paciente en el Impact Hub de Madrid, leyó una carta desde el futuro a un público altamente implicado en la transformación del sistema de salud. Allá por el 2017 el sistema de salud público, los sistemas privados, las empresas de seguros y algunos otros agentes, estaban empeñados en construir y custodiar el mejor historial clínico electrónico para los pacientes. Cada uno de ellos nos aseguraba que la información disponible en el suyo era de vital importancia para nosotros, pero a menudo no la entendíamos. Para convencernos de que su registro era el mejor, permitían incluso introducir algunos datos provenientes de nuestras interacciones con otros sistemas y empezaban a absorber datos de algunos dispositivos y apps de salud; esas que empezábamos a utilizar con mucha ilusión pero que la gran mayoría de nosotros dejábamos de lado, aburridos, unos meses después de haberlas estrenado. Cada uno de esos agentes pensaba que su historia clínica era la mejor, la que más
  • 3. funcionalidad ofrecía, la que más seguridad proporcionaba y la que los pacientes debíamos utilizar, ante todo. Eso sí, los datos ahí disponibles eran tremendamente difíciles de extraer en formatos que no fueran papel o PDF y la integración con otros sistemas, cuando era posible, era realmente precaria y difícil. La estandarización de datos y la interoperabilidad no cogió fuerza hasta que empezamos a trabajar en el registro de salud vital universal. Hasta ese momento a ninguna entidad le interesaba realmente que sus datos pudiesen ser compartidos con otras instituciones. ¡Los datos son de los pacientes! decían, pero la forma de registrarlos y extraerlos, no facilitaba en absoluto la interoperabilidad que nosotros, los pacientes, necesitábamos. Ahora, al nacer, te crean tu registro de salud y se empiezan a volcar ahí todos tus datos de salud. Se ha convertido en un registro universal, que recoge información de millones de ciudadanos de todo el planeta, con lo que hemos conseguido una masa crítica de datos tal que, gracias a la inteligencia artificial, se ha convertido en una herramienta de inteligencia colectiva. Algo realmente útil para la salud del ciudadano, para los profesionales sanitarios, para la sostenibilidad del sistema de salud público, así como para el mundo de la investigación. Esta plataforma nos permite a los ciudadanos con enfermedades crónicas y familias estar en contacto con otros de características similares a las nuestras. El soporte entre pacientes en los diferentes estadios de la enfermedad, es algo que los sistemas sanitarios finalmente han entendido e incorporado como parte de muchos tratamientos. Ya no tengo visitas programadas con el endocrino. Mis interacciones con él, a menudo de carácter virtual, ocurren en momentos concretos, bajo demanda, cuando surge una necesidad especial o cuando mis niveles de glucosa andan alterados. Siempre utilizamos el mismo procedimiento: le envío mis cuestiones previamente y en la reunión, vamos al grano. A veces no es necesario ni la reunión, con los nuevos cursos de realidad virtual, que me prescriben, he sido capaz de resolver muchas de mis dudas, de recibir información sobre nuevos fármacos y dispositivos de nueva generación.
  • 4. A través de la plataforma podemos también ver un registro de todos los ensayos clínicos que se están llevando a cabo a lo largo de todo el planeta. La información, gracias a la realidad virtual, está presentada de forma que la podamos entender. Y no solo eso, podemos también incidir sobre cuáles son nuestras prioridades de investigación, aquello que nos aporta verdadero valor. Podemos ceder nuestro registro histórico de salud y participar en las investigaciones que deseamos. Hoy en día, la mayor parte de los ensayos que se realizan, no se realizan con humanos, sino con simuladores de fisiología humana. Las supercomputadoras y la posibilidad de acceder de forma rápida a miles de registros de salud, entre otros, han acelerado este proceso tremendamente y lo que antes se hacía en años hoy se hace en pocos meses. Hoy podemos decir, que tanto la medicina preventiva como la orientada al diagnóstico y tratamiento, son realmente personalizadas. Nuestra información genética y metabólica está ya cargada en nuestro registro de salud. Parece mentira que hace tan solo diez años recibiésemos tratamientos diseñados para la media (para todos, nos decían, aunque realmente no estaban adaptados a casi nadie). Pero no solo eso, últimamente se están incluyendo también características psicosociales para diseñar nuestros tratamientos óptimos. Pero lo que más me gusta de mi registro de salud vital, es mi “gemela virtual” o “digital twin”. La primera vez que oí hablar de este concepto fue allá por el 2015, y no era para humanos, sino para máquinas y grandes infraestructuras. Se trataba de una tecnología utilizada para emparejar objetos físicos a sus gemelos virtuales, permitiendo así hacer labores de simulación, análisis y predicción de diferentes acontecimientos en los gemelos virtuales. Unos años después se extrapoló ese concepto a un gemelo digital personal. Así pues, mi gemela digital tiene intrínseca toda la información volcada en la plataforma e interactúo a menudo con ella. Está conectada con el resto de gemelos digitales, aprenden entre ellos y de nosotros y esto nos permite a mis médicos y a mi tener acceso a mucha y muy relevante información. Hacemos simulaciones de mi estado de salud en el futuro e incluso comparo mi estado de salud y otra serie de métricas con la de otras personas. Pero lo verdaderamente importante es que ella me ayuda y me motiva a mantener un estilo de vida saludable. Al fin y al cabo, ella es capaz de registrar y procesar datos que para mí es imposible controlar y puedo centrar mis esfuerzos en vivir…
  • 5. Seguro que a estas alturas estás pensando en la humanización, en que todo este mundo digital, ha ido en contra de las relaciones humanas. Pero no nos engañemos, tenemos ahora mucho más tiempo para las relaciones humanas que hace diez años. Porque ¿era acaso humano, toda la burocracia que teníamos que sufrir los pacientes? Las visitas programadas, los desplazamientos, las esperas, pedir y recoger recetas… ¿era humano que los profesionales sanitarios tuviesen que atender a más de 40 pacientes al día, tratando de mantener a flote un sistema de salud diseñado para una realidad que no era la del momento? Ahora, con el registro de salud vital, se han rediseñado y digitalizado muchos procesos, se han eliminado muchas ineficiencias y duplicidades intra y entre organizaciones sanitarias, el liderazgo compartido en salud es una realidad y el sistema sanitario público y privado, vuelven a ser sostenibles. Por último, el nuevo sistema de salud del 2027 es mucho más que reactivo, se trata de un sistema proactivo, enfocado en atender la salud de todos los ciudadanos y la enfermedad de aquellos que la padecen. Por lo tanto, abarca mucho más que la atención sanitaria y se extiende a nuevos agentes, no estrictamente sanitarios. __________________________________________________________ Como veis, las cosas en el 2027 podrían ser muy diferentes. Pero no olvidemos estas tres cosas:  La tecnología disponible en el 2017, ya permite todo lo que describo…  Los principales retos son deconstruir mentalidades y ser capaces de visualizar una realidad diferente sustentada en modelos diferentes…  Nadie será capaz de liderar la transformación necesaria solo… Tenemos la capacidad de convertir los actuales sistemas de salud en unos mucho más inteligentes para todos. ¿Unimos fuerzas?

Thanks to all the patients, healthcare professionals and managers, industry professionals and technologists who have inspired me ... Dear Maria Angeles, Today, February 9, 2027, I am writing this letter to tell you how I manage my chronic condition, how I interact with the health system, how I participate in research; to let you know about the support among patients, standards and interoperability and specially to introduce you to my vital health record ... You will see how the new technologies already in place in 2017 have ended up transforming health systems worldwide, thanks to the mindset change and the transformation of the main actors’ roles (including the patient). I hope that this letter will make us faster in imagining and designing a new health system, and especially in joining forces to achieve it. I have a personal vital health record. This record belongs to me; it is not exclusively linked to any specific health organization, but it interacts with all of them. It is a smart record that automatically absorbs all my interactions with the public health system and other private centers; primary as well as specialized care. In addition, all lab and other sort of tests are loaded there, regardless of who performs them. This information is loaded smartly (not only in text format), and allows me to analyze, for example, what the evolution of my glycosylated hemoglobin is over time, relate it to other indicators and learn a great deal from it. It is also connected to all kinds of devices and wearables: those that measure the glucose, of course, and others that measure, for example, the quality of my sleep, my physical activity and all those parameters that I want to connect. My continuous glucose monitor sends information to the platform from time to time and the pen/pump does it too every time I inject insulin. Whenever my glucose is uncontrolled for more than two days, I receive an alert and a suggestion to take certain corrective actions. As you see, this vital health record gives sense to all those APPs and devices that were previously unconnected and it remarkably potentiates their value.

Every time I eat, I take a picture of the plate with an APP; enter the weight in grams, and considering the amount and sort of carbohydrates of the meal, it suggests me the dose of insulin to inject/infuse. No more surprises like before! Whenever I ate out, calculating the amount of carbohydrates I was consuming and the amount of insulin I had to inject was hard and often only a guess. Of course, all this information is also automatically loaded into my vital health record. The personal vital health record has been made possible thanks to the mobilization organized a few years ago. Tired of the fact that each health system and/or insurance company kept its own disconnected medical record system, some patients and agents of the health ecosystem in Spain, got together to design and build a platform that soon became universal, safe and in which each citizen could have his own integrated record. A record that provides valuable information to us, the citizens, which allows us to maintain or improve our health condition and motivates us to adopt healthy lifestyle choices. The kick-off took place at the second Digital Health Association meeting, held in Madrid on February 9, 2017; when a patient coming from a so-called Cluster for Patient Empowerment at the Impact Hub Madrid and who sounded a bit odd, read a “letter from the future” to an audience highly involved in the transformation of the health system. Back in 2017, public and private health systems, insurance companies and some other organizations, were committed to building and guarding what they called the best electronic medical records (EMR) for patients. Each of them assured us that the information available in their EMR was of vital importance for us, but we often did not understand the information in it. Those EMRs even allowed us to introduce some data from our interactions with other systems and started to absorb data from some health devices and apps; those that we began to use with great enthusiasm but that the great majority of us often put aside, bored, a few months after having started with them. Each of these organizations thought that their EMR was the best, offered the most functionality, more secure and the one that citizens had to use primarily. Of course, the data available in those EMRs was tremendously difficult to extract in a smart format (other than paper or PDF), and the integration with other systems, when possible, was precarious and difficult.
  • 8. Data standardization and interoperability did not gain momentum until we started working on the universal vital health record. Until then, no organization was really interested in having its data shared with other institutions. “Data belongs to the patient!” they said, but the way that data was registered and extracted, did not provide at all the interoperability that we, the patients, needed. Now, every new-born baby gets his vital health record where health data is registered from the beginning. The platform has now grown into a universal platform that collects information from millions of citizens around the world, which has achieved a critical mass of data such that thanks to artificial intelligence, it is now a truly collective intelligence tool. Something extremely useful for citizens, for health professionals, for the sustainability of public health systems, as well as for the research world. This platform allows chronic condition citizens and families to be in contact with peers. Peer support in the different stages of life is something that health systems have finally understood and incorporated as part of many treatments. I no longer have scheduled visits to the endocrinologist. My interactions with him, often of a virtual nature, occur on demand when a special need arises or when my glucose levels are uncontrolled. We always use the same procedure: I send my questions in advance and we get down to the point at the meeting. Sometimes the meeting doesn’t even take place, with the virtual reality courses I get prescribed, I have been able to solve many of my concerns and to receive information on new generation drugs and devices. The platform allows us to localize all the clinical trials that are taking place all over the world. The information, thanks to virtual reality, is offered in a way that we, citizens, can understand. And not only that, we can also influence the research agenda based on our priorities and what brings us true value. If desired, we can grant investigating institutions access to our vital health record and participate in the research we want. Furthermore, today, most of the trials are not performed with humans, but with simulators of human physiology. Supercomputers and the capability to rapidly access thousands of health records, among others, have accelerated investigation processes tremendously and what took years in the past, nowadays is done in a few months.
  • 9. Today we can say that preventive medicine as well as diagnosis and treatment, are truly personalized. Our genetic and metabolic information is already loaded into our vital health record. It is difficult to believe that only ten years ago, we received treatments designed for the average (for everyone, they told us, although they were not really adapted to almost anyone). But not only that, recently, psychosocial characteristics are also being included to design our optimal treatments. But what I like most about my vital health record is my "digital twin". First time I heard about this concept was back in 2015, and it was not meant for humans, but for machines and large infrastructures. It was a technology utilized to match a physical object to its virtual twin, allowing to perform simulations, analysis and predictions of all kinds at the virtual twin environment. A few years later, this concept was extrapolated to a personal digital twin. Thus, my digital twin has intrinsically all the information loaded on the platform and I often interact with her. She is connected to other digital twins, they learn from each other and from us, and this allows my doctors and me to have access to plenty of interesting and useful information. We can make simulations of my health in the future and even compare my health status and other set of metrics with others. But what's really important is that she assists and motivates me to make healthy lifestyle choices. After all, she can record and process data that is impossible for me to control simultaneously and I can focus my efforts on living… Surely, at this point you are thinking of humanization, and that this whole digital world has gone against human relations. But let's be realistic, we now have much more time for human relations than we did ten years ago. Besides, was the bureaucracy that patients had to suffer those days human? Programmed visits, travels to and from locations, waiting lists and time, prescription requests and picking up procedures... was it human that health professionals had to see more than 40 patients a day, trying to sustain a health system designed for a reality that actually didn’t exist anymore. Now, thanks to the vital health record, many processes have been redesigned and digitalized, many inefficiencies and duplications within and between health organizations have been eliminated.   • 10. health is now a reality, public health systems are again sustainable and private ones are back on business. Finally, the new health system in 2027, is not just reactive, it is a proactive system, focused on addressing the health of all citizens and the disease of those that suffer from it. Therefore, it covers much more than healthcare alone; it extends its boundaries incorporating a complete set of new social and industrial agents. -------------------------------------------------------------------------------------------------- As you can see, things in 2027 could be very different compared to now. But let us not forget these three points: • The technology available in 2017, already allows everything described in the letter... • They main challenges are mindset change and being able to visualize a different reality built on different models... • No one will be able to lead the necessary transformation alone ... We have the capacity to transform current health systems into much smarter ones for everyone. Let’s join forces!
